Initial Evaluation
The initial evaluation of a child for Attention Deficit Disorder requires getting information from each parent and teacher. Please click on the links below to print copies of the Parent and Teacher forms (one for each parent and one for each teacher). Once the forms have been completed, please return them to the office. Follow Up Evaluation
We do regular follow up of children diagnosed with Attention Deficit Disorder to make sure they are getting the best care. As part of these follow up appointments, we ask you to have the follow up forms completed by each parent and by each teacher.
