ADD/ADHD Evaluations & Forms

Initial Evaluation
The initial evaluation of a child for Attention Deficit Disorder requires getting information from each parent and teacher.  Please click on the links below to print copies of the Parent and Teacher forms (one for each parent and one for each teacher).  Once the forms have been completed, please return them to the office.  Dr. VanGelderen or Dr. Thill will contact you once the forms have been received and scored.

Follow Up Evaluation
We do regular follow up of children diagnosed with Attention Deficit Disorder to make sure they are getting the best care. As part of these follow up appointments, we ask you to have the follow up forms completed by each parent and by each teacher.  Please click on  the links below to print copies of these, have them completed, and bring them with you to your child's next ADD recheck appointment.

Our Care Management Program specializes in working with patients who have acute, multiple or complex chronic care needs. Areas of focus include ADHD/ADD, autism, asthma, behavioral concerns, delays in development, health and nutritional needs including elevated BMI, and mental health conditions like depression or anxiety.

Remember the best advice to avoid and control infection is to:
  • Avoid close contact with people who are sick.
  • Practice social distancing at all times.
  • Use regular hand washing with soap and water for at least 20 seconds (or use hand sanitizer with at least 60% alcohol if soap and water are not available).
  • Cover your cough with a tissue or the upper part of your arm or sleeve.
  • Wear a mask when leaving home or outdoors (DO NOT mask children under the age of 2).
  • Abide by all local, state, and federal laws and ordinances.
